Top Exceptions (Reviews)
The Report
Introduction
"The purpose of this report is to help providers/partnerships identify service users that appear in the monthly exceptions report as having:
- No TOP
- A TOP that was completed outside of guidelines
- No TOP care coordinator
- More than one TOP care coordinator"
How you can use the data in the reports
After identifying which clients do not have a TOP or had a TOP completed outside of guidelines, it is recommended that providers/partnerships attempt to identify reasons why and put practices in place to overcome the any issues.
If a client has been identified as having no TOP care-coordinator or more than one TOP care-coordinator, it is recommended that the provider/partnership investigates which provider should have had TOP care responsibility and retrospectively correct the care coordination field
Data used
The data used is taken from the Rolling 3 month tables which are located on the PROVIDER tab of the Adult treatment review exception reports. Data was not taken from the PARTNERSHIP & PROVIDER tables as this is grouped by DAT of resident
Please note : The attributor report only includes clients that started treatment after 1st November 2009 and have given the necessary consent, that enables us to share attributor data with partnerships/providers.
Structure of the report
At present, each table shows the following information:

This client has been in treatment for 6 months and ther e is NO review TOP , you are the TOP care coordinator and no other provider is claiming to be the TOPCC.
you will need to check if a review TOP is in the Casefile.

This client has been in treatment for 6 months but did NOT have a start TOP therefore a review TOP is not applicable for performance use.
No Further Action required, this is for information only.

More than one provider is claiming to be the TOP Care Coordinator.
Check this out with the other provider and come to an agreement about who is the TOPCC. If you are unsure who the other provider is , please contact the NDTMS team

Client has had a review TOP done but it is outside of the guidance 5-26 weeks.
Check the TOP date is correct for this client.
